For the Registry of Public Agencies, be sure you have the following information:

  • Full Legal Name of Public Agency
  • Public Agency's County or Counties where an office is maintained
  • Official Mailing Address of Public Agency
  • Name, title, and business address of Chairperson, President, or Other Presiding Officer
  • Name, title, and business address of the Clerk or Secretary
  • Name, title, and business address of the other Members of the Governing Board

For the Amendment to Registry of Public Agencies, be sure to have the following information:

  •  Nature of Update
  •  Full Legal Name of Public Agency
  •  Public Agency's County or Counties where an office is maintained
  •  Official Mailing Address of Public Agency
  •  Name, title, and business address of Chairperson, President, or Other Presiding Officer
  •  Name, title, and business address of the Clerk or Secretary
  •  Name, title, and business address of the other Members of the Governing Board

Registry of Public Agencies filings submitted electronically must purchase a certified copy ($5.00).
